Introduction
Your CTO receives an email on a Tuesday morning: a prospective enterprise client wants a SOC 2 Type II report, a copy of your HIPAA policies, and responses to a 200-question security questionnaire. Due in two weeks.
Your team scrambles. Someone opens the compliance spreadsheet last updated four months ago. Half the tabs are out of date. The encryption configuration evidence is from before the last infrastructure migration. The access control list references three employees who left the company. The incident response plan was written for a product architecture that no longer exists.
This is not a company that ignored compliance. This is a company that did it manually and discovered that manual tracking decays faster than anyone expects.
The patterns below repeat across every healthcare company we work with at Momentum that reaches a certain scale. They are not failures of effort or intent. The Spreadsheet Compliance Model
Most healthcare startups begin compliance management the same way: a spreadsheet. One tab per framework, rows for each control, columns for status, evidence location, last review date, and owner.
This works. For a while.
At one framework, one product, and a team of 10 to 20 people, a spreadsheet is sufficient. The person who built the infrastructure also maintains the compliance documentation. They know where the evidence lives because they produced it. The gap between the spreadsheet and reality is small because the same people are responsible for both.
The model holds until one of these things happens:
- You add a second compliance framework (typically SOC 2 after HIPAA).
- Your infrastructure changes faster than your documentation.
- The person who maintained the spreadsheet changes roles or leaves.
- An auditor or enterprise buyer asks for evidence that is more current than your last quarterly review.
Each of these triggers a different failure mode. Most companies hit all four within 18 months of their first compliance effort.
Five Ways Manual Compliance Breaks
1. Evidence gaps between audits
Manual compliance processes are periodic. You collect evidence quarterly, or before an audit, or when someone remembers. Between those collection points, your environment keeps changing. Deployments happen. Access permissions change. Configuration updates roll out. New services get added.
The evidence you collected three months ago may no longer reflect your current state. When you do collect evidence again, you are also collecting surprises: controls that drifted, configurations that changed without documentation, and third-party tools that updated their defaults.
In a continuous monitoring model, evidence is collected automatically as changes happen. There is no gap because the monitoring never pauses. In a manual model, the gap is the default state.
2. Framework overlap confusion
HIPAA, SOC 2, and GDPR overlap significantly in their security requirements. Encryption at rest satisfies a control in all three frameworks. Access control logging satisfies HIPAA's audit controls, SOC 2's monitoring requirements, and GDPR's accountability obligations.
In theory, this overlap should reduce the total work. In practice, manual tracking makes it harder, not easier.
Each framework describes its requirements in different language, with different control numbering, and different evidence expectations. A spreadsheet with separate tabs per framework often tracks the same control three times with three different descriptions, three different evidence links, and three different review cadences. Changes to the underlying control need to be reflected in all three places. When they are not, you end up with inconsistent compliance status across frameworks: passing for HIPAA but failing for SOC 2 on what is functionally the same control.
Automated platforms solve this by mapping controls across frameworks once and updating all mappings when the underlying control changes. Manual processes require someone to remember, every time, which frameworks share which controls.
3. Personnel dependency
In manual compliance, institutional knowledge lives in people. The engineer who built the infrastructure knows where the encryption keys are managed. The security lead knows which IAM policies map to which compliance controls. The operations manager knows the last time the incident response plan was tested.
When any of these people leave, change roles, or go on leave, the compliance program loses context. The spreadsheet says "Encryption: AES-256, see AWS KMS config" but does not explain why that specific configuration was chosen, which compliance controls it satisfies, or what would break if it changed.
This is not a documentation problem you can solve by writing more documentation. It is a structural problem with manual processes that depend on human memory to connect technical implementation to compliance requirements. 4. Audit preparation costs
Audit preparation in a manual model is a project. It has a start date, a deadline, and a team assigned to it. Engineers stop product work to collect evidence, update documentation, fill gaps, and prepare for auditor interviews.
For a first SOC 2 Type I audit, preparation commonly takes 4 to 8 weeks of part-time engineering effort. For a Type II audit, the preparation window is shorter (the evidence period is longer, but you are still compiling it at the end), but the total overhead across the observation period is higher.
When you add a second framework, the audit preparation windows may overlap. HIPAA reviews, SOC 2 audits, and GDPR assessments operate on different calendars. A company managing three frameworks manually can spend a quarter of its engineering capacity on compliance activities across the year.
With continuous evidence collection, audit preparation shrinks to a review exercise. The evidence already exists, timestamped and organized by framework. The auditor accesses it directly. The engineering team's involvement is limited to answering questions about specific controls, not recreating three months of evidence from memory and logs.
5. Control drift
Control drift happens when your compliance documentation says one thing and your environment does another. A developer disables a security group rule temporarily and forgets to re-enable it. An IAM policy gets broadened during an incident and never gets scoped back down. A new service launches without the standard audit logging configuration.
In a manual model, control drift is invisible between reviews. The spreadsheet still shows a green status for the drifted control because no one has checked it since the last review cycle. The drift compounds: one misconfiguration becomes two, becomes five, becomes a pattern that is expensive to remediate.
Continuous monitoring detects drift within hours, sometimes minutes. An alert fires when a control's actual state diverges from its expected state. Remediation happens while the drift is small and the context is fresh. The team that made the change is still available to explain it, and the fix is usually a revert rather than a multi-day remediation effort.
The Multi-Framework Multiplication Problem
Compliance overhead does not scale linearly with the number of frameworks. This is the math that surprises most teams.
One framework requires a set of controls, evidence collection, documentation, and periodic review. Adding a second framework does not double the work because of control overlap, but it more than doubles the complexity. You now need:
- Cross-framework control mapping (which controls satisfy which requirements in which frameworks).
- Evidence that satisfies both frameworks' formats and expectations (HIPAA wants different evidence artifacts than SOC 2 for functionally similar controls).
- Separate audit timelines, auditor relationships, and preparation cycles.
- Framework-specific documentation (HIPAA policies, SOC 2 policies, and GDPR records of processing activities are all different documents even when they describe the same practices).
- A way to track when a change to one control affects compliance status across all applicable frameworks.
At three frameworks (a common configuration for healthcare companies: HIPAA, SOC 2, GDPR), the cross-framework mapping alone is a significant maintenance burden. A single control change might need to be verified against 10 or more cross-mapped requirements. Missing one creates an inconsistency that surfaces during the next audit as a finding.
This is where the spreadsheet model fails most visibly. The human overhead of maintaining cross-framework consistency in a manual system exceeds the capacity of any single person. Either you hire dedicated compliance staff or you accept that inconsistencies will accumulate. Both options are more expensive than automating the cross-framework mapping.
What the Alternative Looks Like
The alternative is not "do compliance better manually." The alternative is to change the model from periodic manual review to continuous automated monitoring.
In a continuous compliance model:
- Controls are monitored automatically. When a control's state changes, the system detects it and fires an alert.
- Evidence is collected continuously. Every check produces a timestamped evidence artifact stored and organized by framework.
- Cross-framework mapping is maintained by the platform. A change to one control automatically updates its status across all applicable frameworks.
- Audit preparation becomes a review exercise, not a data collection project.
- Personnel changes do not create knowledge gaps because the control-to-requirement mapping is encoded in the system, not in someone's memory.
